Print and complete any new patient forms online or arrive a few minutes early to fill them out in the office.
Bring a valid ID, your insurance card, and a list of any medications you are currently taking.
If you have previous dental records or x-rays, please bring them or have them sent to our office ahead of your appointment. If you give us your previous office information, we can also facilitate this for you. - Your Initial Consultation
